or the hospital you visit.
Can it be done in all the public hospitals?

why pap smears......the s is misplaced here, if you keep your ear open there are normally more than 3 free pap smear tests in the city of nairobi!!! ..."Wewe ni mtu mdogo sana....na mwenye amekuandika pia ni mtu mdogo sana!".

By the way, come to think of it, is there an age limit for these things? I still do mine and mammos annually but may be i need not do them anymore.... and i got a thread doing rounds that now i need to add to that list because of the threatening Ovarian cancer! NKT! I dont know but cancers seem to be on the rise... can we possibly do all these tests or do you just drop them and hope it wont happen? You could be doing for one and you get another.... who knew you can be so careful with life and live so cautiously then get some stupid pancreatic cancer from no where ? Every side i turn, there is cancer this, cancer that, cancer whatever....... its worrying. You can never tell which one hits you and when. It is either i have grown too old and my agemates are ripe for "harvesting" and hence am living on borrowered time or something's just gone bananas with out times... may be the former! But anyhow, enjoy your life when you still have your health.

PAP smears cannot be done in all public hospitals, they just dont have the capacity. PAP requires a brush or spatula, and a slide, which cost about KSH 200, and a sterile speculum, which may cost about ksh 100 at best. It needs a trained health worker who should immediately fix it, label and dispatch to the lab. In the laboratory, this requires staining and microscopy by a trained practitioner. These should be supervised by a pathologist, who should issue a final report. This is because false results in PAP smears can be extremely detrimental to the patient. Most poor results result from poor collection technique and processing which is very common in public hospitals.
Furthermore, PAP is a screening test. Those found with precancer would require treatment. Most public hospitals lack the capacity for this treatment. Furthermore, these specimens from curative surgery need evaluation from a pathologist for proper diagnosis. The government has been toying around to the idea of use of cryotherapy for treatment after screening with visual inspection with acetic acid. the drawback is the potential of undertreating advanced disease while giving a false sense of confidence.
